A heartfelt message to all my patients:

I want to inform you that my allergy practice will be closing on December 21, 2020. I have been in practice at the same office since July 1983. It has been my honor and privilege to help with your health care since that time. I am touching 70 years of age and felt it was time for this change. I have some minor health issues that need to be addressed shortly and I have two children and three wonderful grandchildren that I would like to see more often.

As of January 1, 2021, your medical records will be securely transferred to Dr Fadi Alkhatib at Allergy Institute PC located at 1701 22nd Street, Suite 207, West Des Moines, Iowa 50266. Their telephone number is 515-223-8622 and their fax number is 515-223-5324. I have known Dr. Alkhatib for a relatively short time but have found him to be caring, well-trained and knowledgeable and he wants to do his best to continue the care you have received over the years. Additionally, his office does have satellite locations that me be closer to you than my office is now.

However, if you want your records transferred to another physician, there is enclosed a medical records transfer form that can be filled out and mailed back, faxed or dropped off in our office. These requests would need to be received here by December 21, 2020, as the holidays would limit us to fulfill your request before the end of the year.

Again, it has been an honor to know you as patients and many of you as friends. Thank you for giving me this opportunity. My best wishes to all of you and good health in the future.
